I wonder if usability feedback could be combined with the
context-sensitive help system? It's probably a good time to get
usability feedback when the user has encountered a problem that requires
a trip to the help system.

Maybe the help browser should have an "Ask for help on the internet"
button that allows the user to contact an appropriate newsgroup or
developer, and which sends a copy of the message to the bug tracking
system? System information (version numbers etc.) should be
automatically included at the end of the message - something like
bug-buddy, but for usability issues rather than segfaults.
