Not much that can be bugfixed other than Exchange -- balsa is working
properly. Smith is a perfectly legal address, and if "smit, john
<whatever>" were not treated as two different addresses, it'd cause serious
problems for local delivery.
Fix Exchange to generate
"Smith, John" <whatever>
