Here is a patch wish add some new features:
Command line options when starting balsa from command line:

--compose=MAILADRESS (open balsa's compose window and sets to
field: MAILADRESS)
--open-unread-mailbox (open first unread mailbox)
--open-mailbox=MAILBOXNAME (open named mailbox)
