Banshee will do that (provided you are on Linux, the Windows and OS X
versions of Banshee are behind on hardware support such as media
players)

A feature request for this functionality has been filed, you can add
your email to the CC field to be updated when progress is made.

No idea, I don't remember being able to turn that off entirely. You
could disable the internet access in the preferences to prevent
Banshee from accessing the webservices that provide the artwork, that
should prevent acquisition of the artwork and thus the bandwidth and
disk space usage.
