No, I've always included the 'hicolor-icon-theme' package with the PyGTK
All-in-one installer specifically because it's needed for the icons glade
uses in the toolbox. The glade-3.8 that's included is the same package
I've built and linked to in bug 634978. Stock images also work fine (for
the record, the gtkrc file used in the PyGTK aio installer specifies
gtk-icon-theme-name = "Tango", gtk-fallback-icon-theme = "hicolor").

But remember, GTK+ no longer shows the images by default for all
widgets (ie a GtkButton with use_stock=True and label=gtk-close only
show the text "Close" but not the image that goes with the text), so
that might be giving the impression something doesn't function correctly.

I've not yet experienced crashes with 3.8 (on Windows XP), and this
is the first report of such behavior, so how is Glade being started?
You're using the shortcut in the start menu (in the "Python 2.X PyGTK"
folder) ,right? Also, what Windows version are you using and could you:

- launch the "PyGTK Command Prompt" (it lives in the start menu right
   next to the "Glade UI Designer" shortcut)
- start depends.exe [1] from the "PyGTK Command Prompt", for example
   type C:\depends.exe and then hit the return or enter key
- File>Open>"C:\Python27\Lib\site-packages\gtk-2.0\runtime\bin\glade-3.exe"
- hit F7
- check all the checkboxes, except "Clear the log window"
- click "OK"
- Glade now starts. Do your thing until it crashes
- Back in Dependency Walker click File>Save As...>glade-3.dwi
- Upload the (compressed!) glade-3.dwi file somewhere and let us
   know where we can get it.
