I am able to control braille display by brltty and as far as I saw, Orca

It's possible to somehow override UI widgets and specify what should be

synthesiser?
Is there a clear solution to this? I will be very grateful if somebody can
help directing me, which api or which object I should look on.
I think the best solution for your application is to write an Orca script for
the application. The script would then look out for the specific widgets in
question, and process the widget's contents via Braille only or Speech only
as you prefer... At least I think Orca's scripting is flexible enough to
allow this. If not, its worth adding.
