Basically, it's the code that draws the gtk widgets onto the screen.

If you want a theme that looks sufficiently different to gtk's default appearance that it can't be achieved just by changing icons or tweaking other parameters in a gtkrc file, you can write your own engine to draw things differently, e.g. to have gradient shading on buttons or that sort of thing.
