Referring to application menu

Hello all,
following discussion at the hackfest, some of those present have
decided to use "Open the <app> menu in the top bar and select
<menuitem>." instead of <guiseq> when referring to the app menu. Some
users found the <guiseq> confusing (which was also a bit of a problem
in gnome-terminal), so it makes sense to be more explicit about where
the menu is located.
One of the other options was to use the ▼ as part of the <gui> label
(for example, <gui>gedit ▼</gui>, but that didn't seem to have much
support.
The problem still remains that there is no app menu on some systems,
but the documentation could always be patched downstream in those
cases even though that would be less preferably than coming up with a
way to refer to the app menu in a way which is location-neutral.
