Sounds and ESD volume

Wow, the best way to appreciate the progress GNOME is making is to
step away for a few weeks and then re-sync. Lots of annoying little
details are fixed up. Great Work!
Anyway, I have 3 thoughts on sound to float out there
1) More of a bug: Event sound seems to turn itself off very often
between sessions. Is this a known phenomenon ?
2) It would be nice to set a default volume level for the events,
at least globally. The issue here is that when I have the
system volume cranked listening to x11amp or similar, pulling
down a menu is loud enough to kill my next-door neighbor.
ESD alread has capability to set both stream and sample
volumes (and panning); samples can even be set ahead of
time as I understand it.
3) Along those lines, has anybody considered building an ESD mixer
program to allow stream volumes to be set from a central
place? There would be some issues to deal with (transient
streams appearing and disappearing, for instance) but it
seems to me that it could add some much-needed functionality.
Just some thoughts, wish I had more time for code right now.
