This is the info that I have for my dial-up account
(I have my dial-up working in KDE but I like GNOME much better)
I have taken this info from KPPP

ISP: Dynamic IP Address
DNS: I have the Domain Name and IP Address
Gateway: Default Gateway

this is all of the info that i had to enter into KPPP so I hope this will 
help.
