If the data matrix has n rows and m columns, you need m+1 x limits and n +1 y limits. If you use labels, they will just label the limits, but the implied values will be 1, 2, 3, and so on. With the provided data, assuming that not at all is less than 25%, not very much less that 50% etc…, you'll get a chart like the one attached. Using unequally spaced values will give rectangles with different sizes. Hope this helps.
