This is pretty much the entire point of this thread.  I now consider
O_CLOEXEC as a 'nice to have', but I will not consider it to be a bug if
we fail to do it in some place.

On the other side of things, it is expected that anyone doing
fork()/exec() really -must- go through the close-the-fds song and dance.
