The easiest way to solve the problem is nautilus debugging. Without that
it's really hard to imagine what is going on your system. Although if
you are not familiar with gdb even trace output can help. You can get it
by

killall nautilus && strace nautilus 2>&1 | grep emblem > nautilus.log

Also, could you send us result of

ls /usr/share/icons/gnome/48x48/emblems

Is there file like emblem-OK.icon?
