Hi,
Not much help I know, but I have seen similar behaviour with 14.04 as well.
On boot, I would hear "Screen reader on, login screen frame, screen
reader off."
Alt+Super+S would usually bring it back to life.
I have no idea if the issue is related to what you are seeing with 16.04
or not though.
