I can't reproduce here. After having said:
Try Azure. Container Registry heading level 2
if I press h I hear:
(I don't understand the first phoneme)-ing to top. Container registry heading level one.
and more h key presses say the headers from top to bottom.

This is with orca 3.32.0, Firefox 60.6.1esr.
