I am using courier-imap 3.0.7, and server side threading is supported,
though is doesn't quite work as well as balsa's local mbox threading.

telnet to the IMAP port on your server, and see if THREAD=REFERENCES
appears in the CAPABILITIES line.  If not, make sure IMAP_DISABLETHREADSORT=1
does not appear in your imap config file (/usr/local/etc/courier-imap/imapd
on my FreeBSD mail server)
