There is a patch in the current Red Hat GTK+ RPM that fixes this
problem by checking themes when the first window is mapped for the
app - it will be merged whenever I get around to 1.2.11.
(There may even be a bug on the 1.2.11 milestone about it.)

For 2.0, it isn't a problem at all, since theme switching is
done via a XSETTING, and XSETTING notification doesn't depend
on having a toplevel... the app listens for events on the
xsettings window.
