I think Sasha is right here. I believe Dave made a different stylesheet
for PS output when we put together the Handbook which turned all the URL's
into endnotes to solve this problem of long URL's not fitting on a line
nicely.
If you want PS output, you may want to ask Dave for his stylesheet.
