How are you creating the file chooser dialog? Please read the
following:
http://developer.gnome.org/doc/API/2.0/gtk/GtkFileChooserDialog.html#gtkfilechooserdialog-response-codes
You must use a known response code for your "accept"-type button.

Users don't normally need to go above their home folder in the
hierarchy. "External" folders, such as network mounts, should appear in
the shortcuts list automatically.
You can go to the parent folder by pressing Alt-up, just like in
Nautilus.
