IMHO, the 3|33+ d00ds who MiX FiLe ExTeNsiON CaSE deserve whatever they
get.:-)

But yes, a better solution is needed. Perhaps a "Case insensitive"
checkbox in the Edit/Add dialogs? (Most extensions would then ship with
this option set.)

I will update the suggestion accordingly.
